Vital sign monitoring in phase II 227 3

Hello, I am curious about others' standard of work related to VS monitoring in phase II. Our policy states 'Upon arrival to Phase II and as clinical condition requires. Vital signs and temperature to be obtained within 15 minutes of patient discharge from Phase II.' Staff have expressed concern over the '15 min prior to discharge'. Many of our patients are discharged within 30 minutes of phase II status. What are other facilities doing Thank you

Stir up Regimen in Phase I 711 1

Inquiring about others practice around the stir up regimen. Is your PACU's practice to dim lights once initial assessment complete and allow patients to sleep for extended period of time or do you keep lights on and do full stir up regimen every 15 minutes. Of course exceptions apply such as treating anxiety, extreme pain etc, delirium etc.

Cranial Nerve Assessment 408 2

Curious about those who assess cranial nerves post carotid endarterectomies. How often and for how long do you assess these Currently, we are being instructed to perform them q 15 minutes for 3 hours. I don't see where our Core Curriculum identifies how often/long. I'm just wondering if this is a 'this is how we did it when I started umpteen years ago' thing.
