Curious about those who assess cranial nerves post carotid endarterectomies. How often and for how long do you assess these? Currently, we are being instructed to perform them q 15 minutes for 3 hours. I don't see where our Core Curriculum identifies how often/long. I'm just wondering if this is a "this is how we did it when I started umpteen years ago" thing.

Hi Angela, we do the CN assessment for our patients q 15 x 4, q 30 x 4, then q 1 x 1, then q2 (or sometimes continue with q 1 hr if that is what the surgeon has ordered). I am not sure for how long they are being assessed q 1-2 once they have transferred to the ICU, but I think it is around 24 hours because I think they are usually transferred out of ICU the following day if there are no issues. It is the frequency that our surgeons typically write for. I do not remember if I had found any literature behind that, but we also say that is our guideline/expectation (outside of an order) for charting that was mutually agreed upon by the stroke coordinator, PACU & critical care.

I agree with the above reply. The neuro assessment time frame is surgeon driven. It also depends on if there is any negative deviations.
