Vital sign monitoring in phase II

Hello,

I am curious about others' standard of work related to VS monitoring in phase II. Our policy states "Upon arrival to Phase II and as clinical condition requires. Vital signs and temperature to be obtained within 15 minutes of patient discharge from Phase II."

Staff have expressed concern over the "15 min prior to discharge". Many of our patients are discharged within 30 minutes of phase II status. What are other facilities doing?

We are doing as your facility. The last set has to reflect that the patient has tolerated being in an upright position (sitting/standing). Our patients usually stay at least 30 minutes.
