ASPAN standards address the discharge after IV narcotics and if the patient has had a reversal. I have worked in Phase 1 and 2. The practice varies some in the facilities. The facility that I am at now says 30 minutes after oral pain medication, other facilities have 1 hour as the criteria. I follow the protocol of the facility that I am at, however, I also look at the cumulative dosing that the patient has received with both oral and IV. If the patient is going home late at night, I also take that into consideration. Check with the criteria of your facility. Also, have your documentation reflect the patient's readiness for discharge home. Does this help?
